So does activating spite on Ur-Kek via tactic require him to gain a fatigue? If so, does it also require him to add fatigue when it is triggered from filling the last damage pip? And at that point if he is exausted does that mean he cannot add fatigue, there not able to make the attack?

The answer is that it costs/adds no fatigue to do the Spite attack, it's a free attack and cannot be upgraded to a power attack.
